She will, 18 weeks is young (generally, the rule of thumb is that puppies can 'hold it' for one hour per month of age - thus, a pup 2 months of age should be able to hold it for 2 hours).
Are you sure that you are watching for 'I gotta go!' signals?
Have the bed and bedding been thoroughly cleaned?

Aside from the above, how big is the crate you have her in? Crate when housebreaking should be big enough to stand up, lay down and turn around in. That's all. Use a divider if necessary.

How would you get that scent out of there? I'm not experienced with puppies.

Wash, rinse and repeat for bedding and specialty cleaners for baskets. Metal crates and most plastics can be treated with diluted bleach.

And if your crating setup IS appropriately configured as above, it would be worth a chat with your vet. It's not so uncommon for young puppies to develop UTIs, which can present as incontinence.
